Further explanation of the benefits may be found in the Kaiser brochures, which are available to you, upon request, from the Plan Administrator at no additional cost. To receive the highest level of benefits, Active Self-funded PPO Plan participants may use an Anthem Blue Cross PPO network provider. Kaiser HMO Plan may use Kaiser Permanente doctors and facilities only.

Medicare-eligible Retirees have the choice of three different Medicare Advantage plan providers: Kaiser Permanente / Senior Advantage, HealthNet, and United Health Care / Secure Horizons.
